Privacy Notice

This notice describes how I, Dr Lauren Moss, collect, use and process your personal information, and how, in doing so, I comply with my legal obligation to you. My practice policy is to respect the privacy of my clients, their families and my staff and maintain compliance with POPI.

I collect basic information about you which does include name, address, contact details such as email, cell phone etc.

I will also collect sensitive confidential data in the form of health information, beliefs, ethnicity, and gender during the services I provide to you and or linked to your healthcare through other health providers or third parties such as medical aids.

I maintain records about your health and any treatment or care you have received previously. These records help to provide you with the best possible health care.

Your information may be used within the practice for clinical reflection and supervision to monitor the quality of the service provided.

All employees and possible sub-contractors engaged by my practice are asked to sign a confidential agreement.

Every member of staff in this practice has a legal obligation to keep information about you confidential. I will only use or pass on information about you if others involved in our care have a genuine need for it. I will not disclose your information to any third party without your permission unless there are exceptional circumstances (i.e. life or death situations), where the law requires information to be passed on.

I may share information with 3rd party organisation such as your medical aid. You can consult with them to find out their privacy policies.
